Support Coordinator

This is a really exciting time to join the organisation as additional funding has meant the charity will be recruiting talented people to a number of new roles throughout the New Year.

We have the following roles available:

• Bury - 2 x 28 hour posts
• South Manchester – 2 x 21 hour posts

Position: S996 Stroke Support Coordinator

Location: Homebased, Bury and South Manchester, however, extensive travel will be required as part of this role

Hours: Part-time, 28 hours per week

Salary: Circa £19,496 per annum (FTE circa £24,370 per annum pro rata)

Contract: This is a fixed term contract for 12 months. Our services are contracted, we currently have funding for this contract until 31 March 2024.

The Role

This is an exciting opportunity to be involved in a brand new project in the North West. The project is funded by SQuIRe (Stroke Quality Improvement for Rehabilitation) .

We’re looking for four innovative, passionate and professional individuals with excellent communication and organisational skills to join the brand new Stroke Recovery Services based in the Bury and South Manchester areas. This is an exciting opportunity to work with stroke survivors and their families to support them following stroke.

The service aims to identify and address the needs of stroke survivors and carers across the stroke pathway, by providing a range of innovative support solutions, supporting them to meet their desired outcomes.

About You

The Stroke Support Coordinator will:

• Support new stroke survivors and their carers from hospital discharge into the community.
• Provide personalised information, advice and support.
• Support stroke survivors to make informed lifestyle changes which will help them to prevent further strokes.
• Work collaboratively with NHS colleagues and other areas of the community to make a difference to the lives of people affected by stroke.
• Be instrumental in setting up the new Stroke Recovery service.

This role requires the post holder to travel across the locality to visit people at home and in community settings. Candidates must be able to demonstrate how they can meet this requirement of the role.

To fulfil the role you must you must be resident in the UK and have the right to work in the UK.
